March 1st Deadline for Open Enrollment and Within District Transfer

Have you moved? Please make sure that the school has your correct address on file in the case of an emergency. Bring in, fax or email a copy of your address verification to the school office.

Your child can remain at Park Avenue for the rest of this school year, but you must fill out a Continuation Form, which is available in the office.

If you would like your child to stay at Park Avenue for 2018-2019, you must fill out a Within District Transfer or Open Enrollment form online by the March 1st deadline. The form and additional information are available at this link.
