Kindergarten Sign Up

Kindergarten Sign Up is scheduled for Wednesday, March 11th from 3:00 – 7:30 pm, but parents of current Park Avenue students can pick up the forms now, and return them between now and March 11th.

Children must be 5 years old by September 15, 2015 to start kindergarten in the fall.

Completed forms must be accompanied by the parent’s photo identification, 2 documents proving address, the child’s birth certificate and immunization records.
